The appropriate collection, use and disclosure of patients’ personal health information is fundamental to our day-to-day operations and to patient care.
Protecting the privacy and the confidentiality of patient personal information is important to this clinic.
We strive to provide our patients not only with excellent medical care and service, but we consider any information from our patient confidential and as such it is protected.

This policy shows our commitment to privacy and expresses how the patient privacy is protected. This Privacy Policy relates to all the personal health information held by Serenity Homeopathic Clinic.

Any information related to a patient personal and medical history is defined as her or his Personal Health information.

Our Commitment to Privacy Policy.

You provided the data when you completed the Intake form where you described the reasons you wanted to seek help. You provided your address and phone number and email so we could contact you when needed.

We will not disclose your information, if you wish your information to be released to a different health care provider, you will have to sign a Consent form to allow us to release the required information. A similar Consent form is used only when you agree that case notes and analysis of your case has that potential to make a significant contribution to our homeopathic community. Any personal identifier will be omitted to protect your identity and because they are not relevant the presentation.

Your data is protected, under no circumstances your personal information is passed on to a third partie

Your data and file will be retained for 7 years in case of an adult. For a child the data will be held until that child has become as adult. After that time the data will be shredded. In some circumstance your file may be held for a longer period.
If there is a change of address, or other relevant information, we would appreciate if you could let us know, as it is important to have accurate information.

Your information is maintained in a paper and electronic files. There are a number of measured in place to safeguard the integrity of your information. Our computers are password protected and the data is held locally and not in the “Cloud”.
